We are hiring a Triage Nurse to provide 24/7 telephone-based clinical support to patients. The role involves assessing callers, providing nursing advice, and routing them to the right level of care — whether that's an ER, urgent care, a telemedicine provider, or a scheduled appointment with their primary physician. You'll work within a remote clinical team of nurses, APPs, and physicians, documenting everything in each partner clinic's EMR. Key requirements: active California RN license, a compact multi-state license, 2–5 years of RN experience, and full fluency in both Spanish and English. Telehealth experience is strongly preferred. Full-time and part-time shifts are available across days, evenings, weekdays, and weekends.

Role Overview:

The Triage Nurse is a vital member of our clinical care team alongside advanced practice providers and physicians. In this role, you will practice nursing via telephone using your clinical training, physician guidance, and approved telephone nursing guidelines and protocols. Responsibilities include electronic prescription refills, triaging patients to the most appropriate level of care, providing nursing advice and self-care recommendations, identifying resources, and performing follow-up activities. All duties fall within departmental and organizational guidelines and are within the scope of practice as defined by the Board of Registered Nursing, in compliance with applicable state laws and medical nursing standards of care.


Essential Functions & Responsibilities:

  • Evaluate patients by telephone, provide nursing advice, and triage to the appropriate level of care—including emergency room referral, urgent care, transfer to an Octiva telemedicine provider, or scheduling with the patient's primary physician.
  • Perform telephonic assessments to determine the risk of serious illness or injury and identify the most appropriate next step in care.
  • Follow clinical protocols and guidelines established by Octiva Healthcare and our partner clinics.
  • Access patient medical charts through each clinic's Electronic Medical Record (EMR) to review relevant information and document the treatment plan and advice provided.
  • Perform additional duties as required or assigned.


Qualifications:


Education:

  •  Graduate of an accredited school of nursing (required).
  •  Bachelor of Science in Nursing (BSN) preferred.


Licensure:

  • Active California RN license at the time of application (required).
  • Compact (multi-state) RN license (required).


Experience:

  • 2–5 years of practical RN experience; outpatient experience preferred.
  • 1–2 years in a physician's office or medical office setting preferred.
  • Telehealth or telephone triage experience strongly preferred.


Knowledge & Skills:

  • Proficiency in telephone-based clinical assessment techniques and medical telephone triage.
  • Strong understanding of medical symptomology, urgent/emergent care criteria, and clinical nursing protocols.
  • Fluent in Spanish and English—both written and verbal (required).
  • Skilled in using multiple Electronic Medical Record (EMR) systems.
  • Ability to make quick, sound triage decisions and respond calmly in emergency situations.
